To be eligible for services, a patient must meet the following criteria: Certified by a physician to have an estimated life expectancy of one year or less. Must have a physician’s order to receive hospice services. Must live in Vida Hospice’s coverage area that includes Los Angeles, Riverside, Orange, San Bernardino, or Ventura Counties. Exhibit Symptoms

You or your loved one may exhibit symptoms that may indicate that it is time for hospice care such as: Uncontrolled or increasing pain. Increased shortness of breath. Increased dysphagia (difficulty in swallowing). Frequent infections. Progressive, uncontrolled swelling. Unresolved nausea / vomiting. Oxygen dependence. Progressive weight loss. Mounting kidney & urinary difficulties. Profound weakness & fatigue. Numerous hospitalizations and ER visits. Worsening mobility & function.

Life Limiting Illnesses

If you or your loved one has an advanced case of the following illnesses, it may be time to consult your physician. The earlier Hospice care is started, the more beneficial it can be to patients and their families. Cancers / Tumors / Malignancies Heart Disease Stroke HIV / AIDS Liver Disease Kidney Disease Lung / Pulmonary Diseases – COPD, Pulmonary Fibrosis Failure of Multiple Organ Systems Parkinson’s Disease Neurological Diseases ALS Lou Gehrig’s Disease Dementia Multiple Sclerosis Alzheimer’s Disease

Vida Hospice offers services to patients who have a terminal illness where the goal of treatment is pain management and symptom control with a limited life expectancy as diagnosed by the patient’s physician. Hospice patients do not have to be homebound. Patients, however, must be aware of their condition and reside within the service area.

Financial coverage is available through the Hospice Medicare Benefit and many private insurance companies. No person shall be denied services based on age, race, national origin, disease, handicap, religion, gender, color, or sexual orientation.
